Please read these instructions carefully and completely before submitting your order form. All fields indicated with a * are required in order to complete an order.

1. Please submit ONE order form per dancer.
    

2. If your dancer is in multiple classes, list their first class of the week first and then add all additional classes in the space provided.
    

3. If you are splitting a package between multiple dancers and/or classes, please list the dancer-class-time-photo type in the photo request space.
    

4. If purchasing multiple individual/buddy photos, please indicate which classes you would like for each individual/buddy photo in the photo request space.
    

5. Double check that contact information is correct before clicking 'submit' on the last page of the form.
